Funeral Planning
The death of a loved one understandably causes stress on family and friends; in light of this, Nativity has prepared a ‘worksheet’ you can fill out to help your loved ones through the difficulties of arranging your own Funeral or Memorial service. Simply click the button below to download!
We also encourage those facing end-of-life decisions to consider completing a Physician Orders for Life-Sustaining Treatment (POLST) form, which can be obtained through your physician or the Washington State Medical association. For more information, click here.
